Mass and volume answer different questions
Mass describes how much material is present. In this context, grams, milligrams, and micrograms are units of mass.
Volume describes how much space a liquid occupies. Liters and milliliters are units of volume. A liquid can have a large volume with little dissolved material or a small volume with more material; volume alone does not reveal mass.
The prefixes matter
The prefix milli means one-thousandth. The prefix micro means one-millionth. That makes a milligram 1,000 times larger than a microgram.
Because mg and mcg look similar, a unit error can create a 1,000-fold difference. The number and the unit must always be read together.
Concentration connects mass and volume
Mass concentration is the mass of a substance divided by the volume of the mixture. In plain language: concentration tells us how much material is present in each unit of liquid.
A label reading 5 mg/mL means there are 5 milligrams of the named material per milliliter of the stated mixture. It does not say how many total milligrams are present unless the container’s total volume is also known.
Total amount and concentration are not interchangeable
Two containers can hold the same total mass at different concentrations. They can also have the same concentration but different total amounts if their volumes differ.
That distinction is why clear labeling often presents both the quantity in the total volume and the quantity per milliliter.

Why label format matters
FDA labeling guidance describes confusion between total quantity and quantity per milliliter as a source of medication errors. For many small-volume injection products, the guidance recommends showing quantity per total volume together with quantity per milliliter.
The lesson for research literacy is simple: always identify the total amount, total volume, and concentration as three related but distinct values.
Why the symbols deserve attention
The official SI prefix for micro is the Greek letter µ, while mcg is commonly used in medication contexts. The symbols mg and mcg can still be confused when read quickly.
